COLUMBIA, SC (WIS) - The Columbia Police Department says an investigation into a 3-vehicle incident is underway which led to the death of a 3-month-old infant on Friday.

Police say that around 4:28 p.m. on April 19, a black Ford Expedition was traveling westbound on Percival Road. A silver Chevrolet Trailblazer was stopped for traffic eastbound on Percival Road while a black Nissan Pathfinder was also traveling eastbound on Percival Road.

The driver of the Pathfinder did not notice until the Trailblazer had stopped and attempted to avoid contact, but the passenger front-side of the Pathfinder struck the Trailblazer's rear driver's side before the Pathfinder crossed the double-yellow line into oncoming traffic and struck the Expedition head-on.

The four people inside of the Expedition and the driver of the Pathfinder were all taken to the hospital by EMS.

According to Richland County Coroner Gary Watts, one of the twin male infants, Kevin Danilo Diaz-Carillo, born Jan. 3, 2019, died from blunt force trauma due to not being properly restrained.

The cause of the incident remains under investigation by the CPD Traffic Safety Unit.
