Serious incidents were reported in recent inspections at two local nursing homes, according to the Pennsylvania Department of Health, which fined two homes for incidents reported in earlier inspections.

Lancaster County has 32 nursing homes, which are inspected by the department. The following is a list of nursing home health inspection reports released since late April.

Standard inspections are performed annually, unannounced. Complaint inspections are performed when a complaint about a nursing home is registered with the department.

Deficiencies are ranked from least to most serious, as follows: potential harm, minimal harm, actual harm and immediate jeopardy.

No immediate jeopardy deficiencies were reported. Actual harm deficiency reports are summarized below, and the number of minimal harm and potential harm deficiencies listed.

The current average number of deficiencies per facility is 8.5 in Pennsylvania and 7.9 in the United States, according to Medicare.

Here are the latest reports:

Brethren Village, 3001 Lititz Pike, Lancaster: Standard inspection April 18; one minimal harm deficiency.

Conestoga View, 900 E. King St., Lancaster: Complaint inspection April 25; two minimal harm deficiencies.

Elizabeth Manor Healthcare and Rehabilitation Center, 320 S. Market St., Elizabethtown: Complaint inspections March 18 and April 24; one actual harm deficiency. The report said investigation of a knee fracture showed the resident was transferred by one assistant instead of the two that facility policy required on three occasions. On April 26 the department fined the home $12,500 and put it on a Level 1 provisional license because of an inspection dated Jan. 14. The report on that date said required equipment wasn't used for two residents and one fell from a bed during a procedure with just one staff member assisting instead of the two that should have been, resulting in a fractured femur. A Level 1 provisional license is the first of four stages before a license is revoked, and allows return to a full license if a home meets requirements in six months. 

Garden Spot Village, 433 S. Kinzer Ave., New Holland: Standard inspection May 10; one minimal harm deficiency.

The Gardens at Stevens, 400 Lancaster Ave., Stevens: Complaint inspections April 22 and May 10; one minimal harm deficiency.

Glen at Willow Valley, 675 Willow Valley Square, Lancaster: Standard inspection March 29; no deficiencies.

Hamilton Arms Center, 336 S. West End Ave., Lancaster: Standard and complaint inspection April 26; six minimal harm deficiencies.

Harrison Senior Living of Christiana, 41 Newport Ave., Christiana: Complaint inspection May 1; two minimal harm deficiencies.

Homestead Village Inc., 1800 Village Circle, Lancaster: Inspection April 9; one actual harm deficiency. The report said a resident was transferred with one assistant instead of the two a care plan required, sustained a leg laceration and required antibiotics.

Lancashire Hall, 2829 Lititz Pike, Lancaster: Complaint surveys April 26 and May 10; no deficiencies. 

Manorcare Health Services Lancaster, 100 Abbeyville Road, Lancaster: Complaint inspection March 25; three minimal harm deficiencies. On April 5 the department fined the home $18,250 and put it on a Level 1 provisional license because of an inspection dated Oct. 17, 2018. The report on that date said a diabetic resident was admitted to the hospital because of a hypoglycemic seizure caused by incorrect administration of insulin. A Level 1 provisional license is the first of four stages before a license is revoked, and allows return to a full license if a home meets requirements in six months. 

Masonic Village at Elizabethtown, 1 Masonic Drive, Elizabethtown: Complaint inspection April 9; no deficiencies.

The Mennonite Home, 1520 Harrisburg Pike, Lancaster: Complaint inspection April 11; one minimal harm deficiency.

Rose City Nursing & Rehabilitation at Lancaster, 425 N. Duke St., Lancaster: Complaint inspection April 16; twenty-two minimal harm deficiencies.

St. Anne’s Retirement Community, 3952 Columbia Ave., Columbia: Standard inspection March 18; five minimal harm deficiencies.
