  1. Employee handled dirty equipment then put on gloves without washing hands. Employee handled dirty dishes and equipment, rinsed hands with water then proceeded to handle clean dishes. | Hands washed, changed gloves.
  2. Staff touched ice with bare hands while placing glasses into large ice bin. Staff handled chopped cabbage used for salads with bare hands. | Ice removed from ice bin, food discarded.
  3. Staff rinsed dirty containers and large metal bowls with water then stored away as clean. Staff rinsed knives in hand washing sink at sushi bar without proper wash, rinse and sanitization. | All equipment placed in dishwashing machine for proper cleaning.
  4. Cooked noodles at improper temperatures. | Corrected.

  1. Chicken soup improperly cooled. | Discarded.

  1. Bags of raw shrimp stored over cooked octopus, diced onions and cooked shrimp in cooler. Raw chorizo stored over uncooked fries in fridge. | Rearranged to prevent cross-contamination.
  2. Employee washed and rinsed several dishes without sanitizing. | Corrected.
  3. Broth for shrimp cocktail not kept hot enough. | Reheated.
